Yampa Valley Medical Associates is discontinuing participation in the Primary Care First program effective October 1, 2025 due to our discontinuance of patient care services under Yampa Valley Medical Associates. There are no anticipated changes to services for your ongoing care under the new entity UCHealth Primary Care Steamboat Springs. Please contact your YVMA provider healthcare team during your appointment, through the MyHealthConnection patient portal, or calling 970.879.3327 Option 5 with questions or concerns.
